After you accept admission to ºìÌÒÊÓƵ and , ºìÌÒÊÓƵ will send you an email with information and instructions for setting up your computer account and email.

After you set up your computer account, you can access required forms online.

Below is a list of forms and action items that we ask you to complete in order to start your academic journey. Should you encounter issues completing the forms, please contact the Orientation team

    Identify your academic interests to help ºìÌÒÊÓƵ match you with your first-year faculty adviser.
  • *
    Authorize ºìÌÒÊÓƵ to release detailed information about your college billing account to your parents, other family members, trustees, or banks.

  • Opens on May 1, 2024. This form is required if you have comparable health insurance coverage and want to opt out of the ºìÌÒÊÓƵ plan. If you do not submit a waiver, you will be charged for ºìÌÒÊÓƵ's health insurance.
  • *
    Review ºìÌÒÊÓƵ's immunization requirements and health record requirements. Then, complete required health forms through the and of required vaccines or exemptions (ºìÌÒÊÓƵ requires proof of COVID vaccine primary series/booster and MMR series).
  • *
    Opens on May 1, 2024. Required for all incoming first-year students and transfers covered under our residency requirements. For more information about living on campus, visit the residence life website.

  • Opens on May 1, 2024. Complete to request accommodations for housing. DAR can accept housing accommodations requests past June 1st, however, after that date ability to implement accommodations is dependent on the available housing inventory.

  • Applications opens on May 1. ºìÌÒÊÓƵ's Multicultural Resource Center's Peer Mentor Program is designed to foster a sense of belonging and support among historically underrepresented students and introduce those in their first year at ºìÌÒÊÓƵ to valuable campus resources and relationships. Apply if you are interested in being matched with a peer mentor for your first year at ºìÌÒÊÓƵ. The deadline to apply is extended until June 15.

    Upload your photo for your ºìÌÒÊÓƵ ID card. This is required for move-in.

    Your first enrollment deposit was submitted when you accepted ºìÌÒÊÓƵ’s offer of admission.

    Federal regulations require that students provide a Social Security number or Tax ID so the college can issue a 1098-T tax form. See IRS Publication 970 for more information. If you do not have a Social Security number or Tax ID, or do not wish to provide it, indicate that on this form.

    Outlines the student’s personal responsibility for college charges assessed to their account.

    In IRIS, update your (including your preferred first name, gender, pronouns, and orientation t-shirt size) and (including your mailing address, phone number) and .

  • This form is required if you want to opt out of tuition insurance, which covers 70 percent of the term’s tuition and fees if you withdraw for medical reasons. Learn more about tuition insurance on our website.

  • Complete to request accommodations for classes.

  • Fill out this form at least one week prior to taking your placement exam, to allow time to get your accommodations in place.

  • If you have life-threatening food allergies or require accommodations to participate fully in Orientation activities, please fill out this form as soon as possible. We require three weeks’ notice, if possible, to ensure accommodations are available.
  • Final High School Transcript Due to Admission*
    Your high school can email your final transcript to the admission office or you can mail it.

  • Required for all F-1 and J-1 visa holders. Registration opens on June 1, 2024.

  • Form opens June 1, 2024. Required for all Pre-Orientation and Supplemental Orientation programs and experiences.

    Opens June 1, 2024. Sign up for an on-campus move-in time slot. We recognize that travel plans may not be solidified by July 15, so you can change your response in the form until August 1.

  • Sexual Assault Prevention Training*
    In early July, you will receive an email with instructions about how to complete this online training program.

  • Check your ºìÌÒÊÓƵ email for your tuition statement.
